چکیده مقاله:

Aims: Renal vascular clamping during partial nephrectomy can lead to renal ischemia and kidney damage due to reperfusion injuries, but a surgical technique without any ischemia can eliminate such unwanted effects on kidney function. The present study aimed to demonstrate the feasibility of partial nephrectomy in conditions where the renal artery is ready to be clamped. Patient Information: In this case series study, 24 eligible patients with small solitary renal masses were included in the study between 2013 and 2018. The option of surgery was open partial nephrectomy. During the time corresponding to the hilar clamping in standard techniques, the renal artery was exposed and ready for clamping if needed, and in this situation, resecting the mass was carried out. Findings: The mean tumor resection time was 22.08±2.50 minutes, and the mean procedure time was 92.00±4.60 minutes. Clear cell renal cell carcinoma was seen in 19 patients, papillary renal cell carcinoma in 3, and oncocytoma in 2 patients. There was no positive surgical margin in specimens with cancer. Lower pole, mid zone, and upper pole masses were observed in 14, 2, and 8 cases, respectively. Conclusion: Although this technique can potentially be a dangerous procedure, in a situation in which the renal artery is well exposed and ready to be controlled, especially when the tumor is in Polar Regions, the mass resection can be safely done without any type of ischemia. Therefore, this procedure can be suggested as a viable one in comparison with other related procedures.

Comparison of Laparoscopic Versus Conventional Open Partial Nephrectomy in Dogs

Objective- Laparoscopic partial nephrectomy (LPN) has been proven to be a safe and effective technique in human. This study was conducted to evaluate the feasibility and safety of utilizing laparoscopic partial nephrectomy in dogs and to compare this technique with open routine partial nephrectomy for the first time in Iran. Design- Experimental study. Animals- Sixteen large mixed-breed femal...

Comparison of the loss of renal function after cold ischemia open partial nephrectomy, warm ischemia laparoscopic partial nephrectomy and laparoscopic partial nephrectomy using microwave coagulation.

PURPOSE Nephron sparing surgery is an effective surgical option in patients with renal cell carcinoma. Laparoscopic partial nephrectomy involves clamping and unclamping techniques of the renal vasculature. This study compared the postoperative renal function of partial nephrectomy using an estimation of the glomerular filtration rate (eGFR) for a Japanese population in 3 procedures; open partia...

Mini-Flank Supra-12th Rib Incision for Open Partial Nephrectomy Compared with Laparoscopic Partial Nephrectomy and Traditional Open Partial Nephrectomy

PURPOSE The purpose of this study was to report our approach of partial nephrectomy (PN) using a supra-12th rib mini-flank incision. We compared mini-incision open partial nephrectomy (MI-OPN) with open partial nephrectomy (OPN) and laparoscopic partial nephrectomy (LPN) to verify whether MI-OPN can be an alternative to OPN and LPN. METHODS This was a retrospective single-center study includi...

Zero ischemia laparoscopic partial thulium laser nephrectomy.

BACKGROUND AND PURPOSE Laser technology presents a promising alternative to achieve tumor excision and renal hemostasis with or without hilar occlusion, yet its use in partial nephrectomy has not been significantly evaluated. We prospectively evaluated the thulium:yttrium-aluminum-garnet laser in laparoscopic partial nephrectomy (LPN) in our institution over a 1-year period. PATIENTS AND METH...

Open Partial Nephrectomy for Pediatric Renovascular Hypertension

Pediatric renovascular hypertension is typically managed with revascularization, angioplasty, or radical nephrectomy. We describe the case of a 13-year-old boy with medically refractory renovascular hypertension who presented to our institution after a failed arterial bypass. Subsequent angiography and renin sampling of the segmental renal veins suggested that the lower pole of the kidney was a...

Impact of robotic partial nephrectomy with and without ischemia on renal functions: experience in 34 cases.

OBJECTIVE In this study we aimed to compare renal functions in patients who underwent robotic partial nephrectomy (RPN) with on-clamp and zero- ischemia techniques. MATERIAL AND METHODS Between 2009 and 2015, 12 off-clamp and 22 on-clamp RPN procedures were performed on a total of 34 patients in two centers. The main outcome parameters examined were serum creatinine, and estimated glomerular ...
